Dear HFCR Community:

Holy Family Cristo Rey is partnering with UAB and Luxor Scientific to provide weekly COVID-19 testing at the school. This is an extra effort being made to keep the school community safe and the doors of the school open. Testing will be provided to scholars, staff, and adult volunteers that are often present at the school. Participation is voluntary with a signed consent form. Results will be provided within 24 hours.

The URL links provided below should be used to register and provide consent. Please select the specific link that applies to you according to grade level or staff/ adult volunteer and complete the form. The first time you attempt to complete the form select "I do not have a QR code", after which the registration process will begin.

If you prefer, you may also submit a paper copy of the registration and consent form. These forms are available in the front office or at the links below. There are both child and adult consent forms; please print and sign the one that applies. The child consent form must be signed by the parent/legal guardian of the scholar. Paper copies of consent forms should be submitted to Mrs. Chrysanthea Thomas in person or emailed to cthomas@hfcristorey.org.
